Position Overview:

We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to provide high-quality therapeutic services to patients in their homes. This position offers the opportunity to make a meaningful difference in patients' lives while enjoying the flexibility and autonomy of home-based care.

Key Responsibilities:

Conduct comprehensive speech, language, cognitive, and swallowing assessments in patients' homes.
Develop and implement individualized treatment plans based on physician orders and patient needs.
Provide therapeutic interventions for various conditions including stroke, brain injury, dementia, Parkinson's disease, and other neurological conditions.
Educate patients and family members on therapeutic techniques and home exercise programs.
Document patient progress and maintain accurate clinical records in compliance with Medicare and insurance requirements.
Collaborate with physicians, nurses, and other healthcare team members.
Ensure compliance with all regulatory standards and company policies.

Required Qualifications:

Master's degree in Speech-Language Pathology from an accredited program.
Current state licensure as a Speech-Language Pathologist.
Certificate of Clinical Competence (CCC-SLP) from ASHA preferred.
Previous experience in home health, skilled nursing, or rehabilitation setting preferred.
Valid driver's license and reliable transportation.
Current CPR certification.

Experience with dysphagia evaluation and treatment.
Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively.
Compassionate approach to patient care.
Proficiency with electronic documentation systems.
Bilingual capabilities a plus.
