Comcare Home Health and All Care Home Health is a patient-centered home health agencies dedicated to delivering high-quality, compassionate care to our community. We are seeking a motivated and detail-oriented LVN Home Health Care Coordinator to join our administrative and clinical coordination team.

Position Summary:
The Home Health Care Coordinator (LVN) plays a key role in coordinating patient care, communicating with clinicians and physicians, managing authorizations, scheduling visits, and ensuring documentation compliance. This position requires a strong clinical understanding, high attention to detail, and excellent communication skills.

Key Responsibilities:

Coordinate and schedule Skilled Nursing and Therapy visits based on patient needs and physician orders.
Serve as the main point of contact between clinicians, patients, families, and providers.
Review and assist with clinical documents to ensure completeness and compliance.
Track SOC, ROC, Recertification, DC, and ordered visits to ensure timely service delivery.
Monitor visit notes, wound photo attachments, and route sheet submissions to meet the 48-hour documentation policy.
Assist with obtaining, submitting, and tracking insurance authorizations (Kaiser, LA Care, Carelon, etc.).
Receive and process new referrals; ensure timely follow-up and initiation of care.
Update and maintain accurate records within the EMR system (MyUnity).
Support daily office workflow and coordination needs.

Requirements:

Active LVN License in the State of California (Required)
Current CPR/BLS Certification
Valid Driver’s License & Reliable Transportation
TB Test / Chest X-ray (current per guidelines)
Physical Exam (within required timeframe)
Clear Background Check (Live Scan may be required)
Previous experience in Home Health or Care Coordination is strongly preferred
Strong proficiency with computer systems and EMR platforms (MyUnity experience is a plus)
Excellent verbal and written communication skills
Ability to prioritize tasks and work effectively in a fast-paced environment
