All Care Hospice is currently seeking a compassionate, organized, and dedicated Licensed Vocational Nurse (LVN) to join our team as a Hospice Care Coordinator in our Glendale office.
Position Requirements
- Current California LVN License (required)
- Hospice Care Coordinator experience preferred
- Strong communication, organization, and time management skills
- Ability to coordinate patient care with physicians, nurses, and interdisciplinary team members
- Comfortable working in a fast-paced office environment
- Willing to make patient visits in the field when needed
- Knowledge of hospice regulations and documentation is a plus
Schedule
- Full-Time
- Monday through Friday
